We offer two convenient ways to submit your application, online or paper copy. We prefer the online application in our effort to go paperless.
Online application – Please complete to form below. The fields with * are required. The form can be used for single applicants or by couples. Note that your application is not complete until you hit the submit button and receive the “Thanks for submitting your online application to SAPT! Please do not forget that you will also need to pay your dues of $35 per person.“ message. Payments are accepted at the monthly general meeting or on our payments page.
PDF – Please request a PDF version of the application using our contact form. After filling it out, you can print it and bring it along with your membership dues to an upcoming event listed on the events calendar or send them to us at the address supplied under payments. Please do not mail cash.
